Welcome to the Appointment List Application page. Before completing this application, check with your local indigent appointment committee to confirm they are accepting IDS’s model application form. They may use a different application form and not accept this one.

The Portal provides you with two options for completing the application form.

1. You can elect to download a digital copy of the IDS Model Appointment List Application to complete on your computer or by hand. (Select the button to the left below.)

2. You can complete the IDS Model Appointment List Application online and have a digital copy of your application sent to your email inbox (a .pdf file). The advantage to completing the application online is that your data will be saved and can be changed or updated at any time and resent to you digitally. If you elect to complete the application online, you will be asked which local list(s) you would like to apply for and an application with each list selected will then be generated for you online. Once you have completed the application online hit the Submit button and a digital copy of your application will be sent to your email address and your data saved under your account.

MUST READ: This site only generates a digital copy of your application. You must submit your application to your local Indigent Appointment Committee as they instruct. The Portal does not submit your application to your local committee.
